New Patients

Our clinic is currently taking on new patients. We ask that all new patients complete a New Patient Registration Form before seeing the doctor. Please arrive early to your appointment to allow time to fill out the form or print out the form below and bring with you on the day.

Your first appointment is an opportunity to get to know our team and for us to create a medical file for you.
After your initial appointment we will request that a Transfer Of Medical Records Request Form is completed. This will allow our treating team to have access to your medical information form your previous GP so that they can best provide care to you.

Please note that we cannot offer the below services on an initial appointment. A review appointment will need to be made for any of these.

• Assessments or Plans (Mental Health Care Plans, Chronic Disease Management Plans)
• Procedures
• Prescription of schedule 8 drugs /  drugs of dependence
